You may have seen berberine appearing everywhere lately — in conversations about blood sugar, insulin resistance, metabolic health and weight management. Social media has even given it dramatic nicknames, including “nature’s Ozempic.”

But berberine is not Ozempic, and reducing this fascinating plant compound to a social-media weight-loss trend misses much of the story.

Berberine has been investigated for its potential effects on several interconnected aspects of metabolic health, including blood glucose regulation, insulin sensitivity and lipid metabolism. And unlike many ingredients that suddenly become fashionable online, berberine itself is certainly not new.

What is berberine?

Berberine is a naturally occurring bioactive compound known as an isoquinoline alkaloid. It is found in several plants, including species of Berberis (barberry), goldenseal, Oregon grape, goldthread and tree turmeric.

Plants containing berberine have a long history of use in traditional systems of medicine, including Chinese and Ayurvedic medicine. Modern research has turned its attention to the compound for a different reason: scientists have been investigating how berberine interacts with biological pathways involved in metabolism.

This is where the story becomes particularly interesting.

Why is berberine suddenly so popular?

One reason is the enormous increase in public interest surrounding metabolic health. Insulin resistance, type 2 diabetes, obesity, metabolic syndrome and cardiovascular risk are now central public-health concerns. At the same time, people are becoming increasingly interested in understanding what happens metabolically before a disease diagnosis.

Berberine sits directly within this conversation because research has investigated its effects on several metabolic markers.

In the research

A 2021 review involving 46 studies and 4,158 participants found evidence suggesting beneficial effects on blood glucose, insulin resistance and lipid metabolism among people with type 2 diabetes. Importantly, researchers also identified limitations — including variation between studies and differences in study quality.

More recent research continues to investigate these effects. The important word here is investigate.

Berberine is promising — but a promising botanical compound should not be transformed into a miracle cure.

First, let’s understand insulin

To understand why berberine receives so much attention, it helps to understand insulin. When carbohydrates are digested, glucose enters the bloodstream. The pancreas responds by releasing insulin, a hormone that helps move glucose from the bloodstream into cells where it can be used for energy or stored.

When cells become less responsive to insulin, the pancreas may compensate by producing more of it. This is insulin resistance. For some people, this process can progress for years before blood glucose reaches the diagnostic threshold for type 2 diabetes.

Insulin resistance is also associated with a wider cluster of metabolic issues, including:

  • increased abdominal fat
  • elevated triglycerides
  • abnormal cholesterol patterns
  • difficulty regulating blood glucose
  • increased risk of metabolic syndrome
  • increased risk of developing type 2 diabetes

This is why metabolic health involves considerably more than simply asking whether someone has diabetes.

How does berberine work?

Berberine does not appear to operate through only one pathway. Researchers have investigated several mechanisms through which it may influence metabolism. One of the most discussed involves AMP-activated protein kinase (AMPK).

AMPK acts as an important cellular energy sensor. It helps cells respond to changes in energy availability and participates in processes involving glucose and lipid metabolism. Berberine has been studied for its interaction with AMPK and other metabolic pathways.

However, describing berberine simply as an “AMPK activator” is an oversimplification. Its biological activity appears to involve multiple mechanisms, and research continues to develop.

Berberine and blood glucose

This is one of the most extensively researched areas. A 2024 systematic review and meta-analysis of 50 randomized controlled trials involving 4,150 participants with type 2 diabetes reported improvements in several glucose and lipid measures associated with berberine use.

When berberine was studied alone, researchers reported reductions in fasting plasma glucose and two-hour post-meal glucose, among other markers. When combined with glucose-lowering medication in the included trials, improvements were also reported in markers including HbA1c.

That does not mean someone taking diabetes medication should simply add berberine. The possibility that both can affect blood glucose is precisely why people using glucose-lowering medication should discuss berberine with an appropriate healthcare professional and monitor their glucose appropriately.

Berberine and insulin sensitivity

Blood glucose is only part of the metabolic picture — the way the body responds to insulin matters too. Research has investigated whether berberine may influence insulin sensitivity and insulin-resistance markers, including HOMA-IR.

This is particularly relevant because insulin resistance can exist before a person develops persistently elevated blood glucose. It also explains why conversations about metabolic health increasingly focus on prevention and early intervention, rather than waiting until blood glucose becomes severely abnormal.

Berberine and cholesterol & triglycerides

Glucose metabolism and lipid metabolism are closely connected. Berberine research has therefore also examined total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides.

Several meta-analyses have reported changes in some lipid markers among people taking berberine. A 2024 analysis, for example, reported reductions in LDL cholesterol, total cholesterol and triglycerides in trials examining berberine alone in people with type 2 diabetes. This is one reason berberine has become interesting within the broader metabolic-health conversation, rather than being viewed purely as a “blood sugar supplement.”

What about berberine for weight loss?

This is where marketing often gets ahead of science. Yes, berberine has been studied in relation to body weight and metabolic markers associated with obesity — but calling it a natural equivalent of prescription GLP-1 medication is scientifically misleading.

A review highlighted by the U.S. National Center for Complementary and Integrative Health found reductions in weight and BMI in some studies, particularly with certain doses and durations. However, the NCCIH also notes important limitations: studies have varied considerably, many participants had existing health conditions, and some research had a high risk of bias.

Therefore, the evidence for berberine as a weight-loss supplement is not conclusive. Berberine may have a role within metabolic-health strategies, but it should not be marketed as though swallowing a capsule reproduces the pharmacological action or clinical results of prescription GLP-1 medications.

Berberine is NOT Ozempic

This deserves its own section, because misinformation spreads quickly. Ozempic contains semaglutide, a prescription GLP-1 receptor agonist. Berberine is a plant-derived alkaloid. They are not the same compound, they do not have identical mechanisms, and they should not be expected to produce identical results.

Calling berberine “nature’s Ozempic” may make an attractive headline, but it is not an accurate scientific description.

What is reasonable is to discuss berberine as one of several botanical compounds currently being researched for its potential role in metabolic health.

Does berberine “reverse” diabetes?

No responsible article should promise that a supplement reverses diabetes. Type 2 diabetes is a complex metabolic disease influenced by genetics, body composition, insulin sensitivity, diet, physical activity, sleep, medications and other factors.

Some people with type 2 diabetes can achieve remission, but remission has a specific medical meaning and should be assessed using appropriate clinical criteria. Berberine research is interesting, particularly in relation to glucose control and insulin resistance — but supplementation should never be presented as a guaranteed replacement for prescribed diabetes treatment.

What about berberine and PCOS?

There is another reason berberine has attracted attention among women: the relationship between PCOS and insulin resistance. Not every woman with PCOS has insulin resistance, but it is common in PCOS and can contribute to elevated insulin levels and hormonal disturbances.

Researchers have therefore investigated berberine in women with PCOS, including its possible effects on metabolic and reproductive markers. The research is interesting, but evidence quality varies, and berberine should not be presented as a universal treatment for PCOS.

Is berberine safe?

“Natural” does not mean biologically inactive. And something capable of influencing biological processes can also produce side effects and interactions. Commonly reported adverse effects of berberine are primarily gastrointestinal and may include:

  • nausea
  • abdominal discomfort
  • bloating
  • constipation
  • diarrhea

Berberine can interact with medications. People taking prescription medicines — particularly those affecting blood glucose, or other drugs with known interaction potential — should speak with a healthcare professional or pharmacist before adding berberine. Berberine should not be used during pregnancy or breastfeeding, and should not be given to infants.

More is not automatically better

One of the biggest mistakes in supplement culture is assuming that if an ingredient has benefits, increasing the dose must produce greater benefits. That is not how responsible supplementation works.

Clinical studies have used different formulations, doses and treatment durations. Those differences are one reason research results cannot simply be applied to every supplement on the market. Quality matters. Formulation matters. Dose matters. The individual taking the product matters. And what other medications or supplements that person uses matters.

Berberine should be part of a bigger metabolic conversation

There is no capsule that can replace the fundamentals of metabolic health. Supporting metabolic health may also involve:

  • Food quality. Prioritising adequate protein, fibre, vegetables and minimally processed foods, while considering the quantity and quality of carbohydrates.
  • Movement. Both regular physical activity and resistance exercise can play important roles in glucose utilisation and insulin sensitivity.
  • Muscle mass. Skeletal muscle is an important site of glucose disposal, making muscle health highly relevant to metabolic health.
  • Sleep. Chronic sleep deprivation can negatively influence appetite regulation, glucose metabolism and insulin sensitivity.
  • Stress management. Chronic stress can influence behaviours, sleep and hormonal pathways that affect metabolic health.
  • Appropriate medical monitoring. People with diabetes, prediabetes or significant metabolic risk should know relevant clinical markers and work with appropriate healthcare professionals.

A supplement should complement that foundation — not distract from it.

Research into weight management is less conclusive, and berberine should not be described as a botanical replacement for prescription GLP-1 medications.

Most importantly, metabolic health cannot be reduced to one ingredient. It is the product of multiple interconnected systems — glucose regulation, insulin sensitivity, nutrition, movement, muscle, sleep, stress, genetics and overall lifestyle.
